
Westchester Disabled On the Move is proud to announce that it is now part of New York State’s Social Care Network (SCN) managed by Hudson Valley Care Coalition (HVCC).
What does this mean?
It means that we are recognized by the New York State Department of Health as a community-based organization that can provide those on Medicaid with a myriad of essential services such as safe housing, transportation, nutrition counseling, and listing of local food pantries and a trusted referral to the services we do not directly provide.
As we have always done, our staff can continue to work to assess the needs of people on Medicaid and connect them to appropriate services, but now we are recognized for state referral of clients. We look forward to this new partnership with HVCC and serving more people throughout the Hudson Valley.
This November, we will begin calling families in need, and with a confidential screening, they will be helped to navigate the system for answers. The goal is to get people the services they need. Stay tuned for more information on this new exciting program.
