Nursing Home Transition and Diversion

My Home Not Nursing Home

In a nutshell this program assists with

  • Getting Out of the Nursing Home
  • Preventing Going Into a Nursing Home

Inspired by Lois Curtis, a person with an intellectual disability and plaintiff in Olmstead v. L.C., the landmark case paved the way for programs that give People with Disabilities the option to live in the community with proper support. After spending much of her teens and early 20s in institutions, Ms. Curtis was asked what she wanted. Her reply?

“Get me out of here. Would you please get me out of here? When am I getting out of here?”

My Home Not Nursing Home is designed to support those who WANT TO GET OUT OF THERE! And stay out of there. WDOM believes People with Disabilities have the right to live in the community and not unjustifiably segregated.

If you are not already a Medicaid beneficiary, enrollment assistance is available. This program enhances our other services for transitioning and remaining out of institutions. We partner with other organizations to provide much-needed services, including:

  1. Finding an accessible home in the community
  2. Making the current home accessible
  3. Applying for subsidized rent, if eligible
  4. Service Coordinator assigned to help survive and thrive in the community,
  5. Applying for and Keeping Disability Benefits
  6. Assistance with healthcare insurance appeals for more home care, medical equipment, and supplies

Those who qualify may also receive a welcome home kit with starter supplies for their new home and a smart speaker, such as an Echo Dot (Ask Alexa), to reduce isolation and connect to the world.

Olmstead Housing Subsidy (OHS)

The Olmstead Housing Subsidy (OHS) is a rental subsidy designed to mirror Housing Choice Vouchers, or Section 8, for a specialized population. This rental subsidy is intended to help seniors and people with disabilities leave nursing homes, or prevent nursing home placement. The participant is free to choose any housing that meets the requirement of the program and is not limited to units located in subsidized housing projects.

This service, like all of WDOM’s services, is always free.
